A patient's record is created the moment they first contact the practice — reception enters the basics, and the file grows from there. During the appointment, the dentist opens the record next to the chair and sees the full picture on one screen: medical history, flagged allergies and current medication, previous treatment notes, and attached documents, photos and X-rays, all in chronological order.
